NEW LOCATION: Due to weather damage to Atrium, the testing location has relocated to the DPH Nurse Clinic Space at Treasure Island Gymnasium. 

749 9th Street at Avenue M. 
COVID-19 testing use the 9th Street entrance.

Testing is free, open to all ages. There is no cost. No doctors note, ID or proof of insurance is required. Testing is by appointment only.

Two options to make an appointment:

1. Online: https://lhi.care/covidtesting or

2. By phone (for those without computer access). Call 1-888-634-1123. Enter prompt #1 ("For information regarding testing or to register for a test"). After a brief message is read press prompt #8 to register for a test.
